Social Worker- Mental Health Outreach

Job Description

• Provides outreach to individuals experiencing mental health crises in hope of preventing further deterioration of symptoms
• Assists individuals with accessing the correct level of care, thus reducing the need for higher acuity care
• Connects and supports individuals in accessing area resources to meet identified needs
• Ensures coordination of client mental health services
• Provides time-limited stabilization and case management services to ensure appropriate services are in place to meet identified needs

Knowledge, Skills and Abilities:

• Knowledge of the principles and techniques of interviewing and engagement
• Knowledge of social casework objectives, principles and methods
• Knowledge of crisis de-escalation skills
• Knowledge of community resources
• Knowledge of psychology/mental health/substance use
• Ability to work well in multidisciplinary team.
• Ability to work constructively in the development and coordination of community resources to meet special needs.
• Ability to manage time productively and prioritize tasks.
• Ability to communicate effectively both orally and in writing.

Minimum Qualifications:

A Bachelor’s degree from an accredited college/university with a major in Social Work, Psychology, Sociology or closely related field. 

Full time: 40 hours/week.
