Goodhue County Releases 2026-2028 Community Health Improvement Plan
Goodhue County is proud to release the 2026-2028 Community Health Improvement Plan (CHIP), a community-driven plan designed to improve the health and wellbeing of Goodhue County residents. This plan is the result of months of community engagement, data collection, and meetings with community partners, and is based on the findings in the 2025 Community Health Needs Assessment (CHNA). The CHIP outlines a shared vision and collective strategies to address three priority health areas identified by the community:
- Support mental wellbeing through mental health care and social connection
- Prevent chronic health conditions through active living and healthy eating
- Address chemical health in youth and reduce overdose deaths
“The Community Health Improvement Plan is the community’s plan.” said Ruth Greenslade, Healthy Communities Supervisor at Goodhue County. “These three priority areas are based on the data, the voices of Goodhue County residents, and the expertise of our tremendous partners. The CHIP focuses on making progress in these areas through collaboration. Providing education, training, increasing access, making small changes, starting or sustaining programs – that’s progress through collaboration.”
The CHIP provides a roadmap for how government agencies, schools, nonprofits, and healthcare providers are addressing health priorities over the next three years. Members of the Community Health Assessment Committee oversaw the development of the 2026-2028 CHIP and will monitor the progress made on each strategy. The mission of the Goodhue County Community Health Assessment Committee is to identify health disparities and top health issues for Goodhue County and implement and evaluate strategies, policies, and programs.
