Community Partner Awards

Applications are being accepted for the November 1, 2025 to October 31, 2026 grant cycle. Not only can we provide the funding, but Goodhue County SHIP can provide coaching and support for projects that relate to active living, healthy eating, breastfeeding support, tobacco-free living and mental well-being. The funding can be used to support projects that aim to accomplish a policy, systems, or environmental (PSE) change, which take change beyond a temporary program and influence behaviors long-term by creating an environment where health can thrive. PSE changes can make a big impact on the health of our community by improving the conditions where we live, work and play.

Requirements for Community Partner Awards

Community Partner Awards are available to communities, non-profits, childcare programs, schools, workplaces, senior centers, and organizations for implementing initiative/projects aligned with SHIP goals. Some projects and expenditures may require approval from the Minnesota Department of Health which oversees SHIP.
A 10% partner site match toward the total Community Partner Award is required.
Community Partner Award recipients must participate in pre- and post-evaluation of their initiative/project.
Community Partner Award recipients must submit a report on the results of the initiative/project within 60 days of the completion of their initiative/project, including submitting photos that show people in action.
