How to Apply
You must apply in person using Form DS-11 if at least one of the following is true:
- You are applying for your first U.S. passport
- You are under age 16
- Your previous U.S. passport was issued when you were under age 16
- Your previous U.S. passport was lost, stolen, or damaged
- Your previous U.S. passport was issued more than 15 years ago
If none of the above statements apply to you, you may be eligible to renew using Form DS-82, allowing you to renew by mail and not be charged a processing fee.
Please note: If you are printing your and completing your forms, they must be printed single-sided.
Passport Photos
We take passport photos in our office for a $15 fee or you may bring in your own photo. Please refer to the U.S. Department of State website for photo requirements.
Fees
When applying for a passport, you will be required to pay two separate fees:
- passport fee: check or money order only
- execution fee: cash or check or credit card
