MNCHOICES assessments are available for all residents of the state of Minnesota to help people identify services that support their identified needs. A Care Coordinator will help individuals make informed choices for services that reflect their assessed needs. A Certified MNCHOICES Assessor completes the comprehensive assessment in the person's primary residence.
We serve those living in Goodhue County and MNCHOICES is free of charge.
