Home & Community-Based Services

MNCHOICES

MNCHOICES assessments are available for all residents of the state of Minnesota to help people identify services that support their identified needs. A Care Coordinator will help individuals make informed choices for services that reflect their assessed needs. A Certified MNCHOICES Assessor completes the comprehensive assessment in the person's primary residence.

We serve those living in Goodhue County and MNCHOICES is free of charge.

Services Provided

Home and Community-Based Services staff provide the following:

• Information and education about community resources
• Information about public programs that can help pay for services
• MNCHOICES Assessments
• Transition assistance for people who want to return to the community setting.
