Documents normally needed for financial assistance:
- Any bill or bills, including bill collection documents.
- Debt documents
- Mortgage or rental agreements
- Last two months' bank statements for EVERY bank account
- Last four pay stubs
- Copy of DD214
- Minnesota drivers license or ID
- Copy of marriage license and/or divorce decrees
- Dependent birth certificates and Social Security numbers
- If dental or optical assistance is requested, bring their contact information.
If in doubt, bring it in.
